Where to Go
- Head south from Lincoln Highway (US-38) and Randall Road
- Cross Keslinger Road and the railroad tracks, then turn right on Fargo Blvd.
- Turn right on Kaneville Road
- Turn right on Soderquist Court
- Turn left into the parking lot, park nearest the entrance and enter
What to Bring With You
Please plan to arrive 5 minutes prior to your appointment time, and bring the following materials with you to simplify your check-in process.
- Picture ID (driver’s license, state-issued ID card, military ID, or passport)
- If the patient is a minor, a parent/guardian ID.
- Current Insurance Card(s)
Wear comfortable clothing; shorts will be provided if needed. Lockers are available to store personal items during your visit.
