Anesthesiology
We are dedicated to bringing you the best care at the most affordable cost. As part of this dedication we contract with a third party to keep anesthesia costs low without sacrificing expertise. We work with Stat Anesthesia Specialists Ltd. to provide and maintain an excellent level of care. Ambulatory Anesthesiologists of Chicago may be out of network and may not accept the same insurances as our surgery center. It is your responsibility to verify your insurance coverage with their group. After your surgery, you may receive a separate bill from their company to cover the costs of your surgical anesthesiologist. Workers’ Compensation Patients
Prior to your appointment, you must provide the name and telephone number of the appropriate contact at your employer and a record of your health insurance coverage. We require this information in order to confirm proper billing arrangements.
Financial Arrangements
All co-pays, deductibles and co-insurance are due at the time of your visit or procedure.
We accept cash, checks, MasterCard, Visa, Discover and American Express. We also accept Care Credit.
Self-Pay Patients
If you are paying for services yourself (self-pay), we require a deposit of $200.00 for an evaluation. If you require treatment that exceeds your deposit payment, a representative from our Patient Accounts Department will call you to discuss additional payment arrangements.
